Be sure to include ALL given names, POB, DOB or YOB, places of residence during the US or UK decennial census years. Alleged year of immigration (if appropriate), et al. miscellany, e.g. names of children, religion, military, etc.
Include a personal check or money order for any amount you’d like: $100, $1,000, up to a maximum of $10,000, depending on how far you want the Team to go & how much you’d like the Team to do.
